Sub windows do not appear

Description
In several parts of mythtv the sub-windows do not appear. For example in the setup program, I can go to the channel editor, see my channels but when I hit E the next window does not show. Video Sources from the main menu has a similiar problem.
In the frontend, if I select General from the menu then whatever is at this point also doesn't show.
In all cases, either the screen looks the same OR there are two or three rectangles down the bottom. In the screenshot attached you can see the three blue rectangles when I hit 'E' down the bottom.
Changing the theme does nothing except the colour of the rectangles that appear. It looks a lot like its trying to draw the buttons of the window (e.g. cancel, next etc). Hitting escape makes the rectangles go away and everything else seems to work ok.
windowed or non windowed mode shows the same problem. Windowed the rectangles are half on the screen but they appear too.
