HDTV/DVB recordings sometimes fail

Description
Over the last week, several of my HDTV recording have failed to create a file.
In the attached mythbackend log file, these events are listed:
tennis started recorded at 17:00 -- PVR350 -- success Smallville started recording at 18:58:31 -- HD3000 -- success CSI started recording at 19:58:31 -- HD3000 -- failed to create file.
I have my DVB tuning time-outs set very high right now.
I don't know how to read the log well enough to figure out why the recording is failing.

Don't spend any time on this. There is a chance that my 3rd HD3000 got into a bad state, and that was causing the problem.
Pulling the card out, rebooting, then putting the card back in may have fixed the problem. I should know in a few days.
