minor tweaks to master_iconmap

A few minor changes to the master_iconmap

• Create ITV (i: Independant Television) callsign - used for generic stations that are an ITV affiliate who may not have their own callsign. ITV is it's own network and should not be confused with IND (Independantly owned and operated stations). The ITV prefix should be used for ITV affiliates and the IND prefix should be used for stations that are independantly owned and not affiliated with any nation network. ITV is the new name for the previously named PAX network.

• Move the Seattle and Chicago ITV affiliates to their own callsign and icons so that if they ever are unique, they will pull the unique icons for their own callsigns. These use the ITV- prefix in front of their callsign.

• Fix a typo (.jpg instead of .gif) for a Canadian station.

fyi, I just submitted the logo to lyngsat so it can actually be added to the icon list.

(In [12703]) Apply part of the iconmap patch to close #2934. I see no reason to use the old ITV logo when the KWPX logo is more recent, so I will use that one until lyngsat creates a generic Ion Television entry, which I submitted to them tonight.
