Add ability to toggle audio passthrough during playback

Description
This patch enables the usage scenario that my family likes namely: Most of the time wanting stereo output through the TV (and/or amp), with occasion need for 5.1 output (mainly for DVD playback).
The current way to do this involves entering the setup screens to toggle the AC3/DTS passthrough flags - which is quite inconvienient and not something that is greatly family friendly.
Once applied the usage of passthrough audio can be toggled (for appropriate audio streams) either from the playback menu, or (in theory; not tested) via a mapped key.
It adds a new database setting 'InitPassThrough' that indicates whether passthrough should be enabled by default on the start of playback or not (our preferred configuration is disabled; by default it is enabled); The patch doesn't include a UI for setting this.

I intend to remove the separate passthrough audio device.
What you really want is the ability to change audio device on the fly more so than enabling/disabling passthrough...
