Allow menu items to be hidden, programmatically

Description

Rework MythThemedMenu to allow buttons to be hidden/shown.

Would easily allow things like hiding the Watch TV button, or finally getting the button list down to a reasonable size for flat themes after the MythDVD/MythVideo merge.
