This icon is missing in mythfrontend but appears in mythweb using svn 14495 (9/14/07).

OK i am using original Iulius theme ( i haven't been keeping track of themes). But I fixed it by adding this to ui.xml file:
<image name="watched" draworder="6">
<filename>type/watched.png</filename> <position>300,195</position>
</image>

The icon is there for all the official themes (except maybe minimalist). You don't say which theme you are using.
