I actually accidentally submitted an old diff for #92; this is the rest of what I intended to submit the first time.

The attached patch also further tweaks MythVideo gallery navigation (pageup/pagedown are no-ops if the user is already at the first/last item in the collection).

(In [6877]) Patch from Robert Tsai to perform the parental controls filtering at the mysql level.

This also fixes the page up/down behavior when on the first/last nodes of the gallery.

This closes #96
